FOREWORD BY A PAEDIATRIC INTENSIVIST
The intensive care unit is a place where patients’ (and their family’s) emotions frequently run amok with anger, denial, sadness, guilt and grief. It is a place where positive news is broken with liberating relief, or negative news is broken with anguish, burning tears and crushing heartache. This is where I work and try to keep sanity in the unit. Paediatric residents do postings with us to expose them to the breath of critical illness in children.
Pravin joined the paediatric residency program in KK Women’s and Children’s Hospital, Singapore, in 2016. He is frequently referred to as the resident who has a keen interest in paediatric palliative care. In fact, he made an impression on me the first day we met. I recall we were covering the high dependency unit, a ward for moderately ill children, some of whom may have impending need for intensive care. One patient had a complex cardiac disease for which his family had opted not for surgical correction as the surgical risk was unacceptably high. This child, now nearly a teenager was reaching end stage disease. Even after all these years, his family found it hard to come to terms with the end. At this point, having forgone the surgery in earlier stages of the disease, there was no cure. Death

was inevitably approaching. Offering invasive life support was not going to reverse the disease and most likely will inflict pain, discomfort and distance him from the loving care of his family. However, it is always challenging to help families see this. Knowing his interests and background, I put Pravin in charge of seeing this patient and discussing his care.
