Der Pyruvatkinase-Mangel (PK-Mangel) in den roten Blutkörperchen ist eine genetisch bedingte Krankheit, die sich als hämolytische Anämie äußert. Ein PK-Mangel besteht ein Leben lang, wobei die Symptome leicht bis schwer ausgeprägt sein können. Trotz aktiver Forschungsprogramme, die zu einem immer besseren Verständnis der Pathophysiologie, Ätiologie und Epidemiologie der Erkrankung geführt haben, wissen viele Ärzte nur wenig über den Pyruvatkinase-Mangel.
Das vorliegende Buch bietet einen kompakten Leitfaden zum Pyruvatkinase-Mangel für Allgemeinmediziner, Hämatologen, Onkologen, Kinderärzte, Internisten, Hämatologie-Fachpflegepersonal und Medizinstudenten. Der Pyruvatkinase-Mangel (PK-Mangel) ist der häufigste Enzymdefekt der Glykolyse, über die Erythrozyten Energie gewinnen. Das Enzym Pyruvatkinase ist ein tetrameres Protein, das die Umwandlung von Phosphoenolpyruvat in Pyruvat und damit einen der beiden energiebildenden Schritte der Glykolyse katalysiert.
Der PK-Mangel wurde zum ersten Mal 1961 beschrieben.¹ Die Erkrankung wird autosomal-rezessiv vererbt² und äußert sich als kongenitale nicht-sphärozytische hämolytische Anämie.³
Um die Auswirkungen eines PK-Mangels verstehen zu können, sind Kenntnisse über den Stoffwechsel in roten Blutkörperchen erforderlich.
Stoffwechsel gesunder Erythrozyten
Reife rote Blutkörperchen (Erythrozyten) besitzen keinen Zellkern, keine Ribosomen und keine Mitochondrien. Damit sind sie nicht zur Zellteilung und Proteinsynthese in der Lage und können nicht, wie andere Zelltypen, durch oxidative Phosphorylierung Energie gewinnen. Stattdessen erfolgt die Energiegewinnung in Erythrozyten durch Abbau von Glukose in Pyruvat und Laktat.
Es gibt zwei wichtige Stoffwechselwege in Erythrozyten, bei denen Glukose als Substrat dient (Abbildung 1.1):
• die Glykolyse, über die Energie gebildet wird
• der Pentosephosphatzyklus (auch als Hexosemonophosphatzyklus oder -Shunt bezeichnet), der eine protektive Wirkung hat.

Glykolyse. Unter normalen Bedingungen laufen etwa 90 % des Glukosestoffwechsels in Erythrozyten über die Glykolyse und nur 5-10 % über den Pentosephosphatzyklus.
