Was ist ein Pyruvatkinase-Mangel?
Der Pyruvatkinase-Mangel (PK-Mangel) ist eine seltene genetische Erkrankung, die die roten Blutkörperchen betrifft. Die Erkrankung besteht von Geburt an, auch wenn sie vielleicht erst später im Leben festgestellt wird. Um zu verstehen, wie sich ein PK-Mangel in Ihrem Körper auswirkt, müssen Sie wissen, welche Rolle gesunde rote Blutkörperchen und die Pyruvatkinase spielen und was mit den roten Blutkörperchen geschieht, wenn nicht ausreichend Pyruvatkinase zur Verfügung steht.
Die Rolle von roten Blutkörperchen
Rote Blutkörperchen sind biegsam und haben die Form einer runden Scheibe mit beidseitiger Eindellung (bikonkave Scheibenform). Dank ihrer Biegsamkeit können rote Blutkörperchen auch durch enge Blutgefäße (die Kapillaren) hindurch treten, um den Körper mit Sauerstoff zu versorgen. Gesunde rote Blutkörperchen können sich auch noch durch die kleinsten Kapillaren zwängen.
Die Rolle der Pyruvatkinase
Rote Blutkörperchen gewinnen Energie, indem sie in einem mehrschrittigen Prozess, der so genannten Glykolyse, Glukose (Zucker) in Pyruvat (ein wichtiges Stoffwechselprodukt) und Adenosintriphosphat (ATP, ein energiereiches Molekül) umwandeln.
Die Pyruvatkinase ist ein Enzym, das den letzten Schritt in diesem Prozess ermöglicht. Sie wandelt ein Protein mit dem Namen Phosphoenolpyruvat in Pyruvat und ATP um. Weniger Pyruvatkinase bedeutet weniger ATP und damit weniger Energie, die den roten Blutkörperchen zur Verfügung steht.
Die roten Blutkörperchen benötigen die durch die Glykolyse gewonnene Energie, um ihre normale Form beizubehalten, biegsam zu bleiben und sich vor schädlichen Einwirkungen (Oxidationsschäden) zu schützen. Bei Personen, die in ihrem Körper normale Mengen an Pyruvatkinase bilden, können die roten Blutkörperchen ausreichend ATP produzieren, um durchschnittlich 120 Tage zu überleben.
Abbau von roten Blutkörperchen
Das Zerfallen und der Abbau von roten Blutkörperchen werden Hämolyse genannt. Normalerweise zerfallen die roten Blutkörperchen nach 120 Tagen und werden von der Milz aus dem Blut entfernt.
Rote Blutkörperchen, denen nicht ausreichend Pyruvatkinase zur Verfügung steht, können nicht ausreichend Energie produzieren, um ihre Form zu behalten. Sie zerfallen leichter als gesunde rote Blutkörperchen. Statt 120 Tagen leben rote Blutkörperchen mit PK-Mangel nur wenige Tage bis Wochen.
Das Zerfallen bzw. der Abbau von roten Blutkörperchen (also die Hämolyse) verursachen eine hämolytische Anämie (niedrige Anzahl von roten Blutkörperchen oder niedriger Hämoglobin-Wert) und Gelbsucht (Gelbfärbung der Haut). Die Gelbfärbung ist auf die Substanz Bilirubin zurückzuführen, die beim Zerfallen/Abbau von roten Blutkörperchen freigesetzt wird.
