    It’s a quiet suburban house. A middle-aged man sits watching daytime TV as the doorbell rings. Who’s this calling? Switching off the TV he gets up and walks steadily towards the front door. He is dressed in jeans and a plain shirt. He steps out of the sitting room into the entrance hall towards the front door.

    Opening the door, he is surprised to see a nurse standing on his doorstep. She is petite, slim and attractive with a golden tan. Her hair is long, beyond her shoulders tied at the back but otherwise lose allowing it to drape across one shoulder. She wears a regular modern nurse’s outfit, a blue cotton, button front tunic with short sleeves, matching trousers and sensible shoes. On the floor, at her feet, is a briefcase with an open zip top revealing papers and folders inside.

    Mr Miller?

    Yes? The man looks puzzled.

    I’m your health visitor. Picking up the briefcase, she steps as though to move into the house. I’m here for your treatment.

    The man hesitates, taken by surprise. He does not let the nurse in. I’m sorry. Who are you? Where are you from?

    I’m Nurse Richards, from the Urology Outpatients Therapy Team. She indicates a photo ID on a clip on her tunic pocket.

    I’m sorry, I don’t understand. The man seems confused. I don’t have any appointments.

    Didn’t you receive a notification? The nurse reaches into her briefcase and pulls out a folder. She thinks to herself, this happens all the time, patients not getting their notifications. Flipping it open, she takes out a letter and hands it to the man in the doorway.

    The man looks at the letter, still clearly puzzled. He sees his name at the top and begins to read the three or four paragraphs of text.

    The nurse looks at her watch. Perhaps you would rather we discuss this inside? She picks up her briefcase, clearly ready to step into the house.

    Oh, yes, okay. The man steps to one side to allow the nurse entry. Referring to the letter he adds, I suppose this looks okay.

    Once inside, she explains. You recently had an appointment with Dr Nath, yes? At the urology clinic?

    The man hesitates and mumbles his acknowledgement. Although she is clearly a health professional, he is a little embarrassed to discuss his medical
