    Chapter 1

    The silence of the darkness surrendered to the deafening blare of a siren.

    "Adult male, 22 years old.  Right open tibia fracture immobilized above and below the fracture.

    Blood Pressure 90 over 50.  Score Pulse 40 per minute.  Respiration  6 per minute.  Patient falling and out of consciousness.  ECG monitor.  100% oxygen via non-rebreather mask.  Established two large-bore peripheral I.V. access 0.9% sodium chloride solution.  10/4 IV morphine."

    Where am I? Nash mumbled.

    You're in an ambulance, Mr. Montgomery.  En route to Dallas Trauma Center.

    Trauma ... why?

    You were hit by a car.  You have an open tibia fracture in your right leg.

    No, not my leg, he begged before the darkness came back.

    Next thing he knew, the siren was gone.  So was the voice.  But it had been replaced by another voice, this one just as urgent. 

    "Cephalothin 2 g q6-8h.  Gentamycin 240 mg/d, metronidazole (500 mg q12h) Tetanus prophylaxis. 

    Initiate debridement stat. Redrape the limb.  Transport to surgery.  Prep for intramedullary nail."

    He cracked open his eyes and jolted when he was greeted with blinding lights. 

    Relax, Mr. Montgomery, another voice insisted.  I'm your anesthesiologist.  You won’t feel a thing.

    My ...

    You're going into surgery.  Everything's going to be fine.

    Chapter 2

    Nash.  Oh thank God!

    Finally, a voice he recognized.

    Mom?  Where am I?

    You’re at Dallas Trauma Center, sweetheart.  Your father's here.  And Noah, too.

    He struggled to move.  Everything hurt.  Thanks for coming...

    Of course we came, Nash!  You were hit by a car.

    He struggled to collect his thoughts.  I remember the doctor said something...

    Thank God someone found you as soon as they did, son, his father said.

    Nash turned his head to look over at his father.  Even that took a Herculean effort.

    Where was I?

    Apparently you’d been jogging, his father explained.  Along  Ollinger.

    Ollinger, that’s right, he mumbled, trying to piece together his jagged memories.  Ollinger.

    Nash, don’t try to say too much.  You're just coming out of the anesthesia.

    I'm so ...

    It'll take more than this to slow you down, brother, Noah said.  We're here with you.  Go back to sleep.

    He so much wanted to sleep – but not before he knew exactly what was going on. 

    Noah, tell me what happened.  Please.

    We'll fill you in on the details later dear, his mother insisted.  When the doctor comes in.

    Mom.  I need to ...

    You broke your leg son.  They repaired it in surgery.

    Repaired... he mumbled slowly.  I had surgery?

    Yes, it’s all done, his father said.  We’re just so happy to see that you’re awake.  They feared you might have a skull fracture.  They found you unconscious in the street.  And in the ambulance, you were drifting in and out of consciousness.  Scared the hell out of us.

    He was having trouble moving his lips to match his thoughts.  But I'm OK?

    Of course you are dear, his mother said.  How about I give you a little bit of this water?  Can you take a little sip?

    When he tried to sit up, it stung.  Alerted, he reached for his arm.  "I.V." he mumbled.

    You need it for fluids son.  And for the pain.

    Pain, he repeated softly.  Everything hurts.

    I know, dear, his mom said, stroking his cheek lightly.  Go back to sleep and rest.  We'll be here.

    Mom always knew best.  OK, Mom.  I ... 

    The next thing he knew, a hand touched his arm.  His eyes cracked back open.

    I just need to check your I.V.  Just give me a second, and then you can go back to sleep, a woman said with a smile.

    Are you a nurse? he asked.

    Yup, she smiled.  I’m your nurse.

    The doctor? Nash asked weakly.

    He’ll be here soon.  Now you go on back to sleep.  Your family's here.  They can speak to the doctor.

    No, I ...

    Nurse, can you see if you can get him in here before Nash falls back to sleep? he heard his mother ask.  I think it would be best.  While he's still semi-sedated.

    Of course.  You just hold on, sweetie.

    Nash opened his eyes to his mother.  You're crying Mom.

    I was just so worried about you, Nash.

    But I'm ...

    You'll be fine, Nash.  She squeezed his hand before turning and looking towards the door.  Hello, Dr. Stanton.

    The doctor walked in and studied a monitor next to Nash’s bed.  All thing considered, you're looking good, Nash.  You've only been out of surgery for, let's see, three hours.

    Doctor, can you?

    Explain?

    Nash was alert enough to catch the body language between the doctor and his father.  Just a slight tug, but as far as Nash was concerned, it was a screaming red flag.

    Do you feel you're ready?  How about I come back in the morning?  When you're more coherent.

    Nash attempted to lift his head, but he couldn’t.  It hurt too much.

    Here, let me adjust the bed, his mother said.  Is that better?

    Much, he slurred.  Doctor, please just tell me what happened.

    Very well, Nash.  You were struck by a vehicle.  You sustained a compound fracture to your right tibia.  Thankfully, a passerby came by soon enough that the EMT's were able to stabilize you.

    Stabilize?

    Your vitals were weak, and you were in considerable pain.  You'd lost quite a bit of blood, Dr. Stanton explained.  When you arrived in the E.R., the team quickly irrigated the wound and prepared you for debridement and surgery. 

    Nash’s head was spinning from all of the fancy words.  The doctor smiled, In simple terms, debridement is the procedure we use to remove damaged tissue from an open wound.  The good news is that we initiated the procedure in a timely manner.  For you, that means a vastly improved healing potential for the remaining healthy tissue.

    Nash looked across the room.  His mother was crying.  His father looked as if he'd seen a ghost.  His brother couldn’t even turn around to face him.

    Doctor?

    We secured your fracture internally with a titanium intramedullary nail.  The nail was implanted in the hollow center of the bone, called the medullary cavity.  The nail forms a self-contained internal splint which stabilizes the fracture.  The procedure allows for a precise alignment of the fracture, the doctor explained.  Barring any complications, you can expect faster healing, which means you’ll be able to put weight on your leg sooner.

    The doctor lifted the sheets and revealed Nash's right leg.  You'll be in some pain.  I've ordered pain medication to take care of that.  Right now you're getting it in the I.V.  When you get home, I'll prescribe what you need.

    Nash stared at his swollen bandaged leg.

    When can I walk?

    You'll be going home with crutches.  I am going to recommend physical therapy immediately upon your release.  No weight bearing, though.  Not right away, Dr. Stanton said.  But I want you to begin with assisted movement during your pre-rehab period.  The point is to reduce stiffness prior to the full rehab.  In the pre-stages the physical therapist will begin manual exercises.  Moving your ankle back and forth, bending and straightening your knee, flexing your calf, stretching your achilles tendon.  To answer your exact question, it could be two to six months before you're walking.

    The doctor pulled the sheets back over his leg.  You will need intensive physical therapy.  And a lot of determination.

    He certainly has that, Noah said firmly.

    I don’t want you to worry about a thing, his father said.  "Arrangements are already in motion.  You will have the finest sports
