Description

The majority (98%) of hospice care is provided at the patient's home, whether a private residence, assisted living, nursing home--wherever the patient calls home. In this episode we describe what services can be expected for the person who receives routine home care from hospice.  Routine home care services can include visits by an RN, a nurse aide, a social worker and a spiritual counselor, a volunteer, and a physician or a nurse practitioner.  Medical equipment is provided when necessary to improve quality of life for the patient or caregiver, and medications are also provided.  Listen to this episode to learn more about routine home care, and to learn what to expect from the team that provides the services.  For more information, you can visit our website at www.theheartofhospice.com.  You can also write to us with your questions or comments at host@theheartofhospice.com.   To connect with us you can also follow us on Facebook, Twitter, or Instagram.  Just look for The Heart of Hospice.
