Description

Documentation is critically important in any healthcare setting, and it's no less important in hospice. Every hospice discipline is required to document their care and interventions with patients and family. In this episode we discuss the purpose and power of documentation.   Every discipline must document every interaction they have with a patient or a member of the patient's care team.  This is required by regulation.  The documentation is then placed in the patient's chart and creates the patient's medical record.   To learn more visit our website at The Heart of Hospice for information about hospice, caregiving tips, self care resources, and advance care planning guides.   Please leave a rating or a review of the podcast on iTunes, or your favorite podcast app.  We would love to hear from you, and you can contact us at host@theheartofhospice.com to tell us about your hospice journey, to ask questions, or to offer suggestions for future content.
