#1 I was a family medicine doctor candidate, and I was nervous about interviewing for a job. I explained to the head of the hiring committee that I wanted to be a doctor because I had a voracious intellectual curiosity and a pragmatic persistence.
#2 I told them about my years in Paris, teaching in a high school so I could study medicine at the Sorbonne, and how my fascination with the human brain led me to a master’s degree in neurotoxicology. I explained how incurable optimism kept me afloat as a Schweitzer fellow in Gabon, West Africa.
#3 I told the doctors at the meeting that I wanted to bring rural family medicine to Cane Creek, and that I was ready to take the risk. They seemed relieved that we had addressed the elephant in the room and could move on.
#4 I met Deirdre in the early 2000s at the Governor’s School, a summer camp for gifted high school students in North Carolina. We fell in love, and I convinced her to move in with me one summer at North Carolina’s most notorious mental institution.

On June 28, 2004, in rural Appalachia, a man with my name and my profession strangled his father in the passenger seat of his Toyota Tacoma. Vince Gilmer, a family medicine physician, had been drinking more than usual and making some impulsive decisions.

The police found Dalton Gilmer’s body, still warm, with contusions and a bruised ring around his neck. He had soiled himself. He had been missing all of his fingers. Vince Gilmer had amputated all of his father’s fingers, then laid the body on the side of Good Hope Road and drove home.
