Los primeros biológicos ya han alcanzado la expiración de sus patentes y se han desarrollado versiones de menor costo: los medicamentos biosimilares. La evidencia acumulada en más de diez años de experiencia clínica, de treinta medicamentos biosimilares y más de 700 millones de días/paciente en Europa, muestra que los biosimilares aprobados pueden ser utilizados con igual seguridad y efectividad que los biológicos originales. El ahorro potencial de costos que se puede obtener a través del uso de biosimilares está presionando a los médicos para que prescriban estos biosimilares con preferencia al biológico originador. La terminología es muy importante en el campo de los biológicos y biosimilares, en donde los términos a menudo se usan incorrectamente, lo que causa confusión (ver la página 13 para ejemplos). Las definiciones de estos términos se muestran en la Tabla 1.1.
Biológicos
Los biológicos son macromoléculas grandes, típicamente proteínas. Varían en cuanto a tamaño y complejidad estructural desde proteínas simples como la insulina y hormona de crecimiento hasta moléculas complejas como factores de coagulación y anticuerpos monoclonales, como se muestra en la Figura 1.1. Muchos biológicos, particularmente los anticuerpos monoclonales, son descritos como terapias dirigidas porque han sido diseñados para interactuar con receptores específicos de las células.

Los biológicos se crean en sistemas vivos, como cultivos celulares bacterianos, animales o humanos, o se extraen de organismos completos; no pueden ser sintetizados químicamente. La mayoría de los biológicos son mezclas complejas: mientras que sus estructuras primaria y secundaria son conocidas, es más difícil caracterizarlas a un nivel terciario. Esto contrasta con los medicamentos convencionales de molécula pequeña
que usualmente tienen una estructura única que puede caracterizarse totalmente y que se producen generalmente por síntesis químicas que no son caras y que son sencillas de replicar. La naturaleza compleja de los biológicos, y sus procesos de fabricación, significa que no se pueden crear moléculas idénticas de sus moléculas.
Medicamentos biosimilares:
Un biosimilar, como su nombre lo indica, es muy similar al producto biológico de referencia, pero no es idéntico. Las estructuras aminoácidas primaria y secundaria, la dosificación y vía de administración, son las mismas. Se permiten diferencias en formulación, presentación y en el dispositivo de aplicación, siempre que estas diferencias no afecten la seguridad o la eficacia. Como con todos los medicamentos biológicos, el reto de los fabricantes y reguladores es asegurar que las copias de los biológicos sean lo bastante parecidas al producto de referencia en estructura y función de manera que no tengan diferencias clínicas en la
