Thanks for trudging through the legalese. Now that we’ve got that out of the way, let’s get to some nursing notes to help you feel more comfortable and confident when charting. Although these notes are real life examples from myself and others, I am not an expert in the field of nursing documentation.

When I graduated from nursing school I thought I was ready to go, but after the first month of real nursing work, in a real hospital, with real life and death situations I was ready to quit. The stress was overwhelming, I began to question if I really wanted to do this work.

There are some areas of nursing you can’t control, but one area of nursing I could do something about was note taking. In the beginning, I was anxious about nursing documentation. Doctors and other more experienced nurses were depending on me to communicate accurate patient information. Those expectations coupled with the legal and ethical issues associated documentation caused a lot of sleepless nights.

My education had given me a good foundation, but it in no way prepared me for what I needed when it came to working under my own license without supervision. I had to figure out how to document nursing notes on my own. I began just jotting down notes here and there in a notebook. As I progressed, the notebook became larger and I began to lend my notes to other new nurses.

I put my notes into book form for many different reasons. I started out writing down simple notes that more experienced nurses were using. I realized there was not an easy to use guide with word for word phrases for the common nurse. After I started to put the phrases into a book form, I realized that this could be more than just a personal reference, it could be any
