This book marks the joy of the end of Season 1
of my Podcast, namely Is Pharmacology Difficult Podcast
. Since the past era, humans have always looked forward to and been dependent on plants. Miraculous and magical serendipity lead to the development of most of the plant-derived medicines.
In ancient times, drugs were used raw, with ingredients, amount unknown. The brainy people then once discovered, observed and established the effect of any plant derived material, etc, it was bound to be used repeatedly, irrespective of knowledge of its composition and active moiety.
EFFECT DOMINATED THE MECHANISM
Some such wonderful examples include the use of Coffee (Caffeine) by an Arabian convent who was inspired by some weird behaviour of Goats who fed on Coffee Berries. Belladonna (Beautiful lady) extract from mushrooms were used to dilate eye pupils by Professional poisoners. Chinese herb Ma Huang
was a source of ephedrine to stimulate circulation. Curare was used to paralyse and kill hunted animals for food.Relief of pain was by Morphine (Opium) obtained from Poppy plant juice.Morphine along with similar natural compounds like Cocaine, ethanol were abused due to their addicting potential too!
EVOLUTION OF PHARMACOLOGY
This includes both the old concept of discovery and the modern concept of synthesising drugs.
Drug Discovery is an important process and its study is inevitable.
Drug invention set its feet strong in the field of Synthetic chemistry, i.e. in the Dye industry.
Paul Ehrlich was inspired to establish the presence of chemical receptors in tissues which reacted and fixed
the dyes. The concept of Receptors
was harbingered.
Consistent interest and hard work led to the invention of Arsphenamine, later patented as Salvarsan
. Added benefits of the technique were used for treatment of Syphilis.
Gerhard Domagk worked on another dye, Prontosil
(first chemical sulfonamide
) which was used in treatment of Streptococcal Infections. This effort heralded the waves of Antimicrobial Chemotherapy.
Indomethacin (NSAID) was discovered by executing tests on organisms.
In vitro concepts were highlighted and the process of synthesis and compound testing came into vogue.
The dawn of understanding approach regarding treatment undertaken was heralded by pioneers namely Francois Magendie, Claude Beranard who did few animal experiments to deeply analyse the drug effects. Rudolf Buchheim found the first Pharmacology Institute in Germany.
In the dawn of 19th century, Oswald Schmiedberg with a brilliant team of AJ Clark, P Ehrlich, JJ Abel and many profound students laid the bricks of Fundamentals of pharmacology, no doubt he is known as The Father of Pharmacology
today!
DRUG DISCOVERY & DRUG INVENTION
Continuous, consistent efforts since those times and still ongoing today have helped us all and thrown a great deal of light to understand mechanism, nature, composition , etc of many drugs.
Ligands are created by enabling 3-D structure Based Drug design (SBDD).
Computer Aided Drug Discovery (CADD) , High Throughput screening (HTS) are novel techniques evolving in the 20th century.
DRUG TARGETING
Macromolecules (proteins) are the drug targets which bind to small molecules to treat the diseases.
Small molecules can be Agonists and Antagonists. The former enhance while the latter decrease the influence and final response. Some small molecules can induce irreversible mutations and may serve as Carcinogens. Genomic data availability has led to the advancements in the field of Proteomics i.e. study of translational proteins. This has brought forward the knowledge and identification of heightened or depressed protein levels in certain important pathological conditions.
TARGET VALIDATION
Once the drugs are identified as potential candidates, there are sought much thorough and robust evidences which prove the inherent capability of a drug to treat a disease and be useful therapeutically. This process minimises the chances of a drug failing in clinical trials. One of the common procedures is the use of Chemical probes
. Another method is to silence genes by RNA modification. Knockout mice
and Transgenic mice
are also great alternatives. In the former, gene coding is totally put off and in the latter promoters guide gene expression.
DRUGGABLE TARGETS
Druggability refers to affinity of the target drug to bind with the small molecule. When the new drug can successfully bind to the small molecule, it is referred to as first
