CHAPTER ONE

Acne Face Mapping At-A-Glance

Acne occurs on the face, chest and back because of concentration of sebaceous (oil) glands over these areas. However, there are distinct patterns observed which point towards different diagnoses. This table succinctly summarises the key mimics and distinguishing features one must be aware of.

Specific maskne symptoms compared to a regular breakout

If you answer yes to any of the following, you likely have maskne:

You have no or mild (<5 pimples per month) acne throughout most of your teenage and adult life AND have observed

The onset of acne is within 1-2 months of wearing a face mask

Location of acne breakout is clustered around the O-zone area as opposed to the T-zone in teenage physiological acne and the U-zone in adult hormonal acne

If you have had a similar breakout in the past, but are not quite sure if it's acne, it could also mean that you have a condition known as perioral dermatitis. It is very different from Acne Mechanica in terms of its physiology though it can look quite similar. As its name suggests, perioral means that the rashes or bumps are clustered around the mouth area and is frequently mistaken by doctors (non-dermatologists) as acne. It is also a condition that is triggered by the specific microenvironment of mask-wearing.

When to visit a dermatologist

If you have more than 5 pimples within a month for 2 consecutive months  occurring either within the mask- O-zone or outside the O zone, you may be on the way to developing moderately severe acne

You have tried OTC topicals for acne and do not find that there is a reduction in the number of pimples, frequency of breakouts.

If your pimple is still persistent after 2 weeks, you may have an acne cyst, which requires medical treatment with triamcinolone injection.

You have persistent skin redness, flaking or pus-filled lesions

You have a previous history of a dermatological condition

You have developed a reaction to your OTC pimple cream or cleanser/leave-on acne product

Your maskne is affecting your self-confidence and you are feeling distressed about it
