What is a Glory Hole?

Well if your here reading this book im guessing you already know or have some idea of what a glory hole is and what their used for. The crazy thing is just wondering how a glory hole even came about. One day did some one just decide to take a drill into the stall of a mans rest room and just start drilling away? I dont even know are there glory holes in ladies rest rooms?
