I. Earth: A world in the Five Realm Plexus.
1 Prologue: The Disappearance
Tamara Hellman never came to terms with the disappearance of her baby.
The loss had been hard on her, and not helped by the circumstances surrounding how it happened.
The night Tamaras’ twins were delivered at Queen Charlottes Hospital Hammersmith in London, a staff nurse had been instructed to take one of the babies to the Neonatal Unit on the fourth floor for an observation.
They disappeared shortly afterwards.
Last sighting according to CCTV footage showed them entering the elevator on the ground floor Maternity Wing, but they never came out on the fourth floor.
2 Untimely Birth
Maternity Theatre Five. 23:35hrs.
A Month Earlier.
Dr Bill Branson pushed through double doors into theatre five, and waited momentarily by the door. It was customary theatre etiquette. Having been paged earlier on with a high priority marker on the message, he had rushed in straight from the gas station, he’d been on his way home from a late evening engagement.
A staff nurse scuttled towards him where he stood by the door. In her outstretched hands, she held a set of blue theatre scrubs, hat, and surgical mask on the ready.
What do we have here?
Dr Branson asked, and headed for a hand wash sink.
The patient came in complaining of lower abdominal pains after she’d tripped, and fell.
Dr Josh said. The thirty-four year old was the Senior House Officer (SHO) on duty when Tamara was booked in at Accident and Emergency. She’s been monitored hourly since 9pm. Dilation has progressed since then. It started at two centimeters, it’s now six centimeters. Hence why we’ve had to page you.
Dr Branson scrubbed his forearms, and hands vigorously in turn at the hand wash sink.
What about the probe test?
Dr Josh hesitated momentarily,
Her amniotic Sac is prolapsing through.
Dr Branson dabbed his hands dry, and slipped them into a pair of surgical gloves that’d been held open for him by the staff nurse.
He glanced at the patients’ notes which had been placed on a nearby trolley, and read it for a moment. His brows narrowed as he read the patients’ name.
Tamara Hellman? I know this patient.
he muttered to the Staff nurse, ''But she's only twenty-eight weeks into her pregnancy,''
He walked towards the operating table, where Tamara laid on her back. Earlier, she'd been positioned for cervical dilation examination. And as a precaution she'd also been prepped for assisted delivery, should it be required. An epidural line had also been prepped ready by the on-call anesthetist.
With sanitized gloved hands clasped together, Dr Branson walked towards Tamara's upper half.
"Hello Tamara, It’s Doctor Branson, remember me? How are you feeling today?' Dr Branson asked calmly.
From her position on the bed, Tamara gave a strained smile at him. She wasn't in any pain as such, but nervous nonetheless.
Her mind raced with all sorts of anxious thoughts about the outcome of her episode. Though she'd heard, and had been told about the risk factors with twin pregnancies, she hadn't dwelt much on it. After her first trimester, Dr Branson, - who happened to be the consultant gynecologist at the time-, recommended a cervical sutures procedure, as added precaution. Especially following a previous miscarriage at twenty weeks.
His diagnosis then had been Cervical incompetence.
''Well, I can't say I haven't felt better.''
Dr Branson's surgical mask moved rhythmically, as he smiled back at her.
''I can imagine you have. Try and relax, I just need to quickly examine you."
Dr Branson left her side, and joined the SHO behind the screen veil.
Tamara's eyes darted about nervously. From her position on the operating bed, she'd wondered what was going on. Most of her view was obscured by the veil screen which the nurses had placed over her lower body, from the waist downwards. This shielded her from the doctors who had performed cervical probe checks on her at intervals.
Tensed with anticipation, she flinched slightly as Dr Branson inserted his hand into her, to perform a cervical probe.
Dr Branson’s brows narrowed, as he felt around Tamara’s cervix region with his gloved hand. I can feel amniotic content straining against the sutures.
''I felt a small prolapse earlier on,'' Dr Josh said, ''Which was why I paged you.''
'Good thing you did, it’ll appear the sutures have been dislodged, somehow.''
Dr Branson withdrew his hand, and walked over to Tamara's side.
Tamara studied Dr Branson's expression as he walked over towards her from behind the veil.
''Tamara,'' he said calmly, ''Your amniotic sac is pushing hard against the sutures I put in place at twenty-four weeks. Like I said back then, it was only meant to act as a safeguard support against undue pressure on your cervix as the babies grew. The sutures have come off their position.''
''And, unfortunately, your sac has grown bigger than what your cervix, and the sutures can sustain further.''
Tamara's eyes narrowed somewhat as she seemed to not understand what Dr Branson was talking about. Months earlier, when she had the cervical sutures put in place, Dr Branson had explained to her that he was cautiously optimistic about the procedure. And that the possibility of her being able to keep the sutures in place till full term was doubtful. But it had been highly recommended. If anything, to help progress the pregnancy past its second trimester, and get the babies past the age and stage of viability, which was twenty-four weeks gestation. He’d also explained the implications of the babies reaching stage of viability, as it had a strong bearing on the survival factors for the babies, and whether the hospital would be obliged to actively intervene.
''So, what does that mean?'' Tamara said.
''The sutures will have to be removed.''
''Are you going to put in another set of sutures in place?''
''No Tamara, the pregnancy is too far gone for that. Your amniotic sac is exposed, and there is a high risk of infection, and other complication if left as it is."
"I’m afraid, your babies will have to be delivered.''
''But it's too early.''
Dr Branson exhaled empathetically.
''Now, remember we talked about this possibility months ago at your review consultation meeting. We did agree that it was part of the prognosis option, considering your peculiar risk factors.''
Tamara thought for a moment as she recollected.
'Yes, I do remember.''
On the plus side, at least the babies are past the age of viability concerns.
Dr Branson said. That has significantly improved their prognosis after delivery. But we'll just have to play it by the day.''
Dr Branson paused.
''Another good thing is, we have one of the best Neonatal Units in the world. So, you needn't worry about the quality of care that your babies will be receiving here.''
Tamara nodded. Reluctantly, she said, ''Okay,''
Dr Branson's mask moved as he smiled at her. ''Good."
"You have to stay positive, and we'll all be rooting for your babies, as we wish them all the best wishes.''

Dr Branson motioned to his SHO. ''Forceps please.''
Dr Josh handed him a pair of Forceps from an array of surgical instruments neatly arranged on a trolley tray.
Carefully, Dr Branson inserted a gloved hand, and felt for her cervix, amongst bulging amniotic sac content which had prolapsed into her upper vaginal cavity. Locating the nylon sutures, he snipped them in turn with the canal scissors. As he snipped the last suture, the dark grey tip of Tamara’s amniotic sac bulged outwards through the birth canal, and onto the mat.
Swiftly, Dr Branson lacerated the sac open, and freed the first baby from its moist enclosure.
''Quickly, get the second baby out,'' he said to Dr Josh, as he cleared away mucus from the baby's airways, and turned it over on its belly. Gently, he massaged the baby's back to stimulate breathing reflexes.

Dr Branson cast a curious look at baby two.
''I'm a bit concerned about her breathing,
He turned towards the staff nurse, ''Take the baby up to the Neonatal Unit, I want her observed. If no improvement, ask them to page me."
I'll see to that right away.
