One in four couples experiences infertility in developing countries. And it has shown an almost similar prevalence worldwide due to improved technology, late marriages, increased consumption of highly processed foods, environmental toxins, and a host of other changes our world today is experiencing.
As a reproductive endocrinologist/IVF consultant for over four decades and I have seen couples or intending parents walk into my office for support in assisted reproduction most times, after which they have seemingly exhausted all options of natural conception. We seem to have forgotten about ourselves being the most important when it comes to reproduction, and we tend to chase something outside ourselves for that to happen. For the purpose of this book, we shall adopt a definition of
‘infertility’ as set forth in the guidelines of the Royal College of Obstetricians and Gynaecologists (UK): failure to conceive after regular sexual intercourse for two years in the absence of known reproductive pathology
.Infertility is the most important reproductive health problem in developing countries, with the lifetime prevalence rate increasing from 6.6% to 32.6%. Infertility can affect one’s whole life and it is associated with dysthymia, anxiety and more diseases. However, in most developing countries, reproductive health care is synonymous with family planning. Attention is given only to the decreasing number of births in these countries but infertility care is given little or no attention.¹
Let’s take Nigeria as a case study. The country has over 193 million people as at the beginning of 2018 (the number was around 190 million in 2017). Yet about 20-25 percent of local couples are childless. This 20-25 percent prevalence of infertility in Nigeria is for couples that are officially married. Still, these numbers could be higher as medical institutions report their own statistics. It is societally expected that a woman must give birth to her own children. Infertility between couples is generally ascribed to the female, albeit erroneously.
A study was carried out in one of the states In Nigeria, with a population of about 3.5 million (according to the last national population census). In the state, there are eight general hospitals, functional infertility clinics, two teaching hospitals, organized Obstetrics and Gynaecology departments with human resources to manage infertility in the state. The study was a descriptive cross-sectional survey among infertile women. A total of 250 respondents returned completely filled questionnaires, giving a response rate of 97.3%. The results revealed that the 25- 34 age group constitutes the highest (44.4%) amongst other age groups. One hundred and ninety (76.0%) of these respondents were married, 47.6% of respondents are skilled workers. This shows that a good percentage of women struggling with infertility are urban skilled working/ business married women
One of the reliable studies² also reveals that the major cause of infertility in Nigeria is infection: sexually transmitted diseases (STDs), postabortal and puerperal sepsis, and these problems are by no means restricted to women. However, male infertility is regarded as taboo, a problem that no one will admit exists. This taboo itself is a contributor to practices of polygyny, with women all too frequently assumed by the local population to be the primary culprit in- infertile marriages, and male infertility handled with discretion, to protect male dignity.
This book considered previous studies in Nigeria which focused on the possible contributions of environmental factors, such as diet and toxic elements, sociocultural behaviours and sociodemographic factors, infections, and hormones. Another study also acknowledges that "irrespective of source, health care development in Nigeria is largely limited to curative services, with prevention, promotion, rehabilitation and other aspects receiving attention only on paper.³ Infertility as a problem faces stiff competition for available resources in this context. Given that
current health spending in most low-income countries is insufficient for the achievement of the health MDGs,"⁴ it is debatable that infertility reasonably has a place in public sector financing, though it has a place in privately financed options via established fertility intervention units like SoPrecious Fertility Intervention Centres.
In the case of Nigeria, this would include an increase in per capita expenditure within the health system from US$9.44 to US$34 as recommended by the WHO. Of course, as former WHO Director-General, Dr Lee Jong-wook, conceded, The MDGs do not say everything that needs to be said about health and development
– and he specifically mentioned reproductive health as one of the needs not included in the MDGs. The data showed that urban women in Nigeria do not desire to stop childbearing until they have reached an advanced age and have a relatively large number of children. These findings are reflective of a high fertility setting where pro-natalist attitudes are prevalent and are reminiscent of what other studies in Nigeria have suggested.³These studies and more reveal that most causes of maternal deaths can be prevented by improved management of pregnancy-related complications. Maternal nutrition plays a crucial role in influencing fertility, fetal development, birth outcomes, and breast milk composition. During the critical window of time from conception through the initiation of complementary feeding, the nutrition of the mother is the nutrition of the offspring – and a mother’s dietary and lifestyle choices can affect both the early health status and lifelong disease risk of the offspring.
