    So what’s it gonna be, Doc? Do I get to break out the champagne, or should I start making out my will?

    One glimpse at the middle-aged oncologist said it all. He replied sadly, I’m so sorry, Clarice. As we suspected, your cancer is no longer contained in the lungs. You have what they refer to as brain metastases.

    Trying her damnedest to steady her suddenly quivering lips that were severely riddled with mouth sores from her excessive chemo treatments, she jokingly piped, Not my genius brain, too! Well, I will have none of that. My mind shall remain as alert as ever. I will continue to put out my literary masterpieces to my adoring fans.

    Doctor Rayborne allowed Clarice her brief moment of denial. His line of work never got any easier in the telling. He was acutely aware of one thing: Each and every patient needed and deserved to retain his or her own level of emotional dignity.

    Tears welled up in Clarice’s expressive blue eyes and, not surprisingly, in the doctor’s as well.

    Exactly how long do I have?

    He grimaced. I think the onset of a month at most. It could be even…shorter. We can continue with your palliative treatment to minimize your symptoms and improve your comfort level.

    She sighed. You mean more lung drainage?

    Yes. The pleurodesis procedure has worked well on you, but now that you are terminal, it will only…prolong the inevitable.

    His somber patient gulped. I really do not wish to prolong any of this. What’s the point?

    Dr. Rayborne reached out and patted her gently on the shoulder. The point is to ease your pain level. We’ve given you radiation therapy for your chest and spine pain. Now, for brain metastases, corticosteroid treatment is recommended. This therapy works very well to reduce any speech problems, headaches, or seizures that you may experience.

    She scoffed, Wow, I have all that to look forward to? I don’t know; I may just have to pass on all of this.

    Doctor Rayborne looked her directly in the eye. These treatments are available if you want them. You are not obliged. I only want to help you.

    The woebegone patient met and held his intense gaze. "How long do I really have, Doc?"
