Orally bioavailable CDK4 and 6 inhibitors have become established standard-of-care options in HR+/HER2- metastatic breast cancer, and these agents are now expected to have a major impact in early breast cancer (EBC). CDK4 and 6 inhibition has recently shown significant benefit as adjuvant therapy in patients at high risk of early recurrence, but other studies in the adjuvant and neoadjuvant settings are under way as well. How should these advances be transitioned to the clinic based on the accumulating supportive evidence? How can surgeons and oncologists work together to ensure proper risk assessment and treatment planning to improve outcomes in patients with HR+/HER2- EBC?
