Digestive System Malignancies - Constantine P. Spanos
Chapter 1: Esophageal cancer
Abstract
Esophageal cancer is the eighth commonest solid organ tumor worldwide and the sixth most common cause of cancer death. There are two main histologic types of esophageal cancer: squamous cell cancer (SCC) and adenocarcinoma (AC). Globally, SCC is the most common type; the incidence of adenocarcinoma has surpassed SCC in Western countries. When esophageal cancer presents with clinical symptoms, it is usually at an advanced stage. Esophageal cancer is locally advanced, metastatic or incurable in 50%–80% of cases on presentation. The Siewert classification is used for tumors in the vicinity of the EG junction. They encompass all tumors with an epicenter within 5 cm proximal/distal to the EG junction; all are adenocarcinomas. Pretreatment staging is an essential step in the management of esophageal cancer. Therapy is based on pretreatment staging assessment. Treatment incudes esophagectomy, endoscopic resection, and esophageal-sparing modalities (endoscopic mucosal ablation, radiofrequency ablation, photodynamic therapy, and cryotherapy). Neoadjuvant therapy (chemotherapy + radiation) is employed in locally advanced
