The United Nations has declared its highest level of emergency in Iraq as a humanitarian crisis follows the rapid advance of Islamic State militants. There have been eye-witness accounts of people beheaded, of whole families buried alive, and there are an estimated 1.2 million Iraqis internally displaced.
