Description

In 2018, state health departments received funding to support diabetes and cardiovascular disease prevention and management efforts under a 5-year cooperative agreement with the Centers for Disease Control and Prevention (CDC). The Alaska Department of Health and Social Services Section of Chronic Disease Prevention and Health Promotion partnered with the Alaska Pharmacists Association (AKPhA) in funding the Sustainable Education and Training Model under Pharmacist Provider Reimbursement (SETMuPP) program. This podcast episode summarizes the initial accomplishments and provides a framework for other groups invested in provision and sustainability of pharmacist-provided health services.
